1. Agreement to These Terms
By accessing our website or booking an appointment with Premium IV Therapy LLC, whether online, by
phone, or through any booking platform, you agree to these Terms and Conditions. Please read them
before booking. If you do not agree, please do not book.
2. Our Services
Premium IV Therapy provides elective IV hydration therapy, vitamin and nutrient infusions, and related
wellness services. Our services are wellness-oriented and are not intended to diagnose, treat, cure, or
prevent any medical condition. They are not a substitute for emergency medical care and should not
replace professional medical advice.
All services require completion of a health intake and consent form prior to treatment. We reserve the
right to decline service to any individual when we believe doing so is in the interest of health or safety.
3. Cancellation Policy
In-Center Appointments: Please contact us as soon as possible if you need to cancel or reschedule. We
appreciate advance notice so we can offer your time to another client.
Mobile Appointments: Because mobile services involve a nurse traveling to your location, cancellations
made within 2 hours of your scheduled appointment, or after a nurse has already been dispatched, may
result in a $50 cancellation fee to cover the nurse's time and travel.
The same $50 fee may apply to no-shows for mobile appointments where a nurse has been dispatched
and cannot be recalled in time.
We understand that life happens. Cancellation fees are applied at our discretion and we will always try
to work with you reasonably. However, repeated late cancellations or no-shows may affect your ability
to book future appointments.
4. Payment
All services are cash-pay and must be paid in full at the time of service. We do not accept health
insurance. Accepted payment methods are posted at the time of booking and may include credit card,
debit card, and other available options.
Prices are subject to change. The price shown at the time of booking is the price you will be charged for
that appointment.
All services are final and non-refundable.
5. Consent & Health Forms
Before receiving any service, you will be required to complete a health history questionnaire and sign
our Informed Consent and Service Agreement. That document covers your rights, the risks of IV therapy,
your health disclosures, and your authorization for treatment. These Terms do not replace that
agreement — both apply.
6. Nursing Services
IV therapy services are performed by independent contractor Registered Nurses (RNs) who maintain
their own professional licensure and malpractice insurance. Premium IV Therapy LLC coordinates your
service experience but is not a licensed medical practice.
7. Limitation of Liability
To the fullest extent permitted by law, Premium IV Therapy LLC's liability for any claim arising from your
use of our services is limited to the amount you paid for the specific service in question. We are not
liable for indirect, incidental, or consequential damages nor any adverse effects resulting from
incomplete or inaccurate information provided by the customer.
8. Governing Law
These Terms are governed by the laws of the State of Arizona. Any disputes that cannot be resolved
informally will be submitted to binding arbitration in Maricopa County, Arizona, unless eligible for small
claims court.
9. General Use
By using our website, you confirm that you are at least 18 years old or accessing the site under the
supervision of a parent or guardian. You agree to use the website and services only for lawful purposes
and in compliance with all applicable laws and regulations.
10. Intellectual Property
All content on this website, including text, graphics, logos, and images, is the property of Premium IV
Therapy or its licensors and is protected by copyright and trademark laws. Unauthorized use of our
intellectual property is prohibited. The text on our website, however, can be used by AI to build its
knowledge base.
11. Changes to These Terms
We may update these Terms at any time. Updated Terms will be posted to our website with a revised
effective date. Booking an appointment after an update constitutes your acceptance of the revised
Terms.
